Very thoughtful design & contents for personal home/car/travel use
Local county health department, hospital & senior center provide free emergency info data sheets & folders, but this product has significantly more fields for information and tags & storage pockets to alert first responders to where info is stored. The package came with five copies of the data sheet, so I have copies on the refrigerator door, in my car glove box, and in my billfold and passport/vaccination card wallet.I bought extra packets to give to family members who live alone.

Great to have on hand.
I know some areas give these out for free, but after calling all the agencies in my area, had to purchase these. Very good package. Not only for your fridge but your car as well. Even one for a pet, if needed. Holds a lot of information needed in case of an emergency. So when an EMT needs the info, you don't have to try to remember, it's all written down.

Keep 2 Copies
It's a good way to keep a list of my medications readily available. Seems that we are prescribed more meds as we age and it's important to have an accurate list when you go to the doctor. I made a second copy of my medications for my wife to carry in case something happens to me and she needs to answer questions on what I've been prescribed.

I have multiple medical issues and take quite a lot of medication. So this really helped me to remember my meds when I'm at the Dr's and emergency room. It also gives my family comfort knowing my information is right on the fridge.

For Emergency Room Visits
I'm afraid I haven't had a chance to put this ER personnel kit to use...yet. Of course, that's a good thing, cuz I'm not quite ready to be "called Home" (although I nearly succumbed to a heart attack in 2004 and a stroke in 2016, as well). I guess I really need to get a good idea of how to use these... just in case! Must be prepared for anything, right?

Smart Solution to the Old Age Problem
I live alone so this is a potentially life-saving purchase.For under $20, I now know that in case of emergency, the EMT's have all the information they need to take care of me.I love that it includes a decal for my door to tell EMT's where the vital information can be found.I can't tell you what a comfort this is.

Get Organized!
This collection of vinyl emergency notification folders, pouches, and holders is a great way to get organized. We keep one of the larger folders in the glove box of each vehicle containing emergency contact information just in case we ever became completely incapacitated or were killed in an accident. We also keep one of the medium sized zip-lock pouches in our freezer and carry the wallet size holders containing the same information.They're nicely made and of very good quality.
